INSTRUCTIONS: Before you get started, we are required by Stanford University's Institutional Review Board to obtain your consent to participate. The information below will explain the details of this study. By clicking on the Continue button below, you are consenting to voluntarily participate in this study.
DESCRIPTION: You are invited to participate in a research study about executive stress and longevity. This study consists of collecting anonymous information using online questionnaires and collecting one blood sample or at-home saliva sample. You may choose to participate in one or both components of the study.
In the online survey, you will be asked questions about various aspects of your work, stress levels, and personal lives. The questions are completely voluntary and you may choose not to answer questions if you wish. All responses are anonymous and confidential; only aggregate results will be reported. If you choose to provide your contact information at the end of the survey, you will be contacted by a research coordinator to schedule a blood draw or arrange to have a salivary collection kit mailed to your home. Your responses on the online survey and the blood draw/saliva results will provide information on executive stress and genetic predictors of longevity.
TIME INVOLVEMENT: Your participation in this experiment will take approximately 10 to 15 minutes. However, this will depend on how quickly you read and answer the questions (i.e., some people need less while others need more time). There are no time limits and there is no obligation to complete the survey.
RISKS AND BENEFITS: There are risks, discomforts, and inconveniences associated with any research study. These deserve careful thought. The risks associated with this study are feeling uncomfortable when answering the questionnaires, being uneasy with the answers you provide, and feeling uncomfortable with the information being asked. The benefits that may result from this study include becoming more aware of various career and life topics that might affect you. We cannot and do not guarantee or promise that you will receive any benefits from this study. Your decision whether or not to participate in this study will not affect your employment/medical care.
PAYMENTS: You will not receive any payment for your participation in the online survey. If you choose to have your blood drawn or provide a saliva sample at the end of the survey, you will be paid $50 for your time.
SUBJECT'S RIGHTS: If you have read this form and have decided to participate in this project, please understand your participation is voluntary and you have the right to withdraw your consent or discontinue participation at any time without penalty or loss of benefits to which you are otherwise entitled. You have the right to refuse to answer particular questions. Your individual privacy will be maintained in all published and written data resulting from the study.
